Duchamp's grave is in Rouen (you're right). He's buried on the Cimitiere Monumentale overseeing the city. The cemetery is huge and the grave not easy to find (I once spent hours looking for it).
When you go through the main entrance, it's somewhere to your far left. But the person in charge will gladly show you. If you go, make sure to call ahead for opening hours.
